Rapid Response Nurse

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at University Hospitals of Morecambe Bay NHS Trust. As a Rapid Response Nurse, you will play a critical role in providing high-quality, patient-centered care to individuals in need of urgent medical attention in the community.
Key Responsibilities- Deliver holistic assessments and develop robust care plans for patients with complex needs.
- Work collaboratively with multidisciplinary teams to ensure seamless care transitions and effective discharge planning.
- Provide expert clinical guidance and support to patients, families, and carers to promote independence and self-management.
- Contribute to the development and implementation of clinical policies and procedures to improve patient outcomes and safety.
- Participate in quality improvement initiatives and contribute to the continuous evaluation and improvement of services.
- Registered Nurse with a valid NMC registration.
- Post-registration education and experience in acute community care.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work under pressure and prioritize tasks effectively.
- Computer literacy and experience with electronic patient records.
