Your Responsibilities

  • Conduct comprehensive assessments of individuals with diabetes, tailoring treatment plans to their specific needs.
  • Deliver nurse-led clinics and collaborate closely with multidisciplinary teams across hospital and community environments.
  • Provide expert guidance and support to healthcare professionals on evidence-based diabetes care practices.
  • Lead structured education groups, empowering patients with the knowledge and skills for effective self-management.
  • Deliver formal and informal diabetes education programs to a range of healthcare professionals.
  • Contribute to the development of integrated diabetes care pathways across primary and secondary care settings.

About You

You will be a registered nurse with a recognized post-registration qualification in diabetes. Your experience should demonstrate your ability to work autonomously, lead teams, and deliver high-quality patient care.
