Your Responsibilities
- Conduct comprehensive assessments of individuals with diabetes, tailoring treatment plans to their specific needs.
- Deliver nurse-led clinics and collaborate closely with multidisciplinary teams across hospital and community environments.
- Provide expert guidance and support to healthcare professionals on evidence-based diabetes care practices.

- Deliver formal and informal diabetes education programs to a range of healthcare professionals.
- Contribute to the development of integrated diabetes care pathways across primary and secondary care settings.

About You
You will be a registered nurse with a recognized post-registration qualification in diabetes.
