District Nurse Team Leader

We are seeking a highly skilled and experienced District Nurse to join our Out of Hours District Nursing Team as a Team Leader. The successful candidate will be responsible for leading a team of District Nurses and providing high-quality patient care in a fast-paced and dynamic environment.
Key Responsibilities- Lead a team of District Nurses and provide clinical guidance and support
- Develop and implement effective care plans to meet the needs of patients
- Work closely with other healthcare professionals to ensure seamless care delivery
- Manage a caseload of complex patients and provide high-quality assessment and treatment
- Develop and maintain relationships with other service providers to ensure effective care coordination
- Registered Nurse with significant recent community experience
- Community Specialist Practitioner qualification - District Nursing
- Minimum 2 years post-qualification experience
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
