Role Overview

This position involves working with adults facing a range of complex needs, including Learning Disabilities, Autism, Mental Health issues, or Physical Health challenges. The ideal candidate will possess a genuine interest in relationship-focused social work, emphasizing the development of independence through a positive risk approach and a strengths-based framework.

You will engage closely with adults residing in supported living environments and residential care settings. Collaborating with these individuals, their families, advocates, and service providers, you will conduct Care Act assessments aimed at fostering independence and personal growth.

We seek a Community Support Specialist eager to join a team dedicated to relationship-centered practices and promoting independence through a constructive risk management approach. This role involves case management and conducting reviews of supported living arrangements, necessitating strong partnerships with various stakeholders, including health services and legal entities.

This position is part of our Countywide team, supporting adults across diverse regions. The role is primarily community and home-based, with flexibility to work from various locations for administrative tasks. Travel throughout the county is required, and you may need to work from different facilities as necessary.

Candidate Profile

To qualify, you should be a certified social worker with current registration. A minimum of one year of post-qualification experience is essential, along with evidence of ongoing professional development and the ability to practice in line with established professional standards.

Attention to detail is crucial, as is familiarity with the Care Act, Mental Health Act, Mental Capacity Act, and related legal procedures. The successful candidate will demonstrate confidence in person-centered approaches and positive risk management.

Additional qualifications include:

  • Demonstrated commitment to ongoing professional development and adherence to current professional standards.
  • Comprehensive understanding of the financial aspects of commissioned care services.
  • Ability to make informed decisions while identifying and mitigating potential risks associated with interventions.

This role requires travel throughout the county. A valid driving license and access to a vehicle, or the ability to meet mobility requirements through alternative means, are essential.
