Youth Support Consultant - Care Transition Team

Hourly Rate: £14 - £21

We Do Social Work is seeking a dedicated Youth Support Consultant to join our Care Transition Team, focusing on assisting young individuals aged 16-25 who have experienced care.


Key Responsibilities:

  1. Fulfill statutory obligations and responsibilities for young individuals in accordance with relevant legislation.
  2. Manage a caseload effectively while maintaining precise records of interactions and interventions.
  3. Contribute to the Pathway Plan Assessment process and provide continuous evaluations to guide Pathway Planning.
  4. Conduct assessments for young individuals seeking to continue their education or training, or those presenting with additional support needs.
  5. Engage with young individuals to promote their well-being, ensuring their perspectives are considered in assessments and Pathway Plans.
  6. Offer practical guidance, assistance, and support tailored to the needs of young individuals.
  7. Identify and coordinate support services for young individuals residing in the community, overseeing the implementation of tasks as per the pathway plan.
  8. Prepare and present Pathway Plans collaboratively with young individuals.
