We are seeking a skilled and compassionate Community Mental Health Nurse to join our multidisciplinary Older People's Community Mental Health Team. As a key member of our team, you will work collaboratively with colleagues to provide high-quality psychosocial and psychological interventions, supporting individuals in their homes and communities.

Key Responsibilities
  • Provide evidence-based mental health care to older adults, utilizing a strengths-based and social inclusive approach.
  • Work in partnership with individuals, families, and carers to develop and implement personalized care plans.
  • Contribute to the development and implementation of the LINCS Model, promoting effective care coordination and collaboration.
  • Participate in the physical health clinic at the ECT suite, addressing the physical health needs of individuals with mental health conditions.
  • Supervise junior members of staff, providing guidance and support to ensure high-quality care delivery.
  • Fulfill the CMHT Duty Worker role, ensuring seamless service delivery and responding to urgent care needs.
Requirements

To be successful in this role, you must be registered as a Mental Health Nurse with the UK NMC Register. You should possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced, dynamic environment.
