PK Health & Social Care Partnership is seeking a skilled Community Care Assistant to join our Wellbeing Support Team.

About the Role

We are looking for a dedicated individual to contribute to assessment, risk assessment, personal outcome and recovery focussed care planning and review. The successful candidate will be directly involved in the delivery of support to adults with mild to moderate mental health needs living in rural towns and villages in Perth and Kinross.

Key Responsibilities
  1. Plan, organise and manage the delivery of support in line with policy, procedure and best practice.
  2. Deliver short-term, recovery-focused support to encourage the development of self-management skills.
  3. Contribute to assessment, risk assessment, personal outcome and recovery focussed care planning and review.
  4. Work collaboratively with colleagues and the community to provide effective support.
Requirements

The successful post holder will require:

  • Qualification to SVQ3/HNC in Health and Social Care level.
  • Previous experience in mental health support work.
  • A valid UK driving licence.
