Job Overview

We are seeking a highly organized and skilled Support Coordinator to become a vital part of our dedicated Care Team, ensuring the provision of exceptional home care services for our clients.

The role of a Support Coordinator encompasses the delivery of superior care support services, which includes assessing and reviewing individual care plans, conducting practical support visits, performing competency evaluations, and providing training for our care professionals. Due to the nature of the position, a valid driving license is required.

Additionally, you will be tasked with identifying opportunities for service enhancement and business expansion.

Key Responsibilities

  • Oversee essential office administration while being responsive to business needs.
  • Facilitate the service delivery process by efficiently constructing schedules that accommodate both clients and care professionals, while remaining adaptable to last-minute changes.
  • Ensure adequate coverage for care assistant absences, including sickness and holidays.
  • Input and maintain comprehensive records of clients and care professionals in our care planning system.
  • Manage service inquiries by responding to emails and phone calls in a warm, professional, and timely manner.
  • Maintain effective systems to ensure all filing and databases are compliant and current.
  • Collaborate with the Registered Care Manager to implement new care packages, including coordinating introductions of care staff to clients.
  • Work closely with the Care Manager and Recruitment Officer to maintain sufficient staffing levels for both new and existing care packages.
  • Coordinate client introductions, competency assessments, and support visits for the care team in the community.
  • Support the delivery of high-quality services through quality assurance calls and client visits.
  • Assist in the recruitment process and pre-employment checks for new care professionals as necessary.
  • Participate in the On-Call Rota and be prepared to provide care to ensure service continuity if needed.
  • Perform any other duties deemed essential for the successful operation of the business.
  • Ensure compliance with Home Instead's policies on Equality, Diversity, and Equal Opportunities in both employment and service delivery.

Salary: £23,000 - £26,000 Pro Rata

Position Type: Full-time

  • SVQ Level 3 Qualification (or willingness to pursue it).
