Royal Voluntary Service is seeking a dedicated individual to fill the role of Care Services Coordinator. This position is part-time and permanent, offering a competitive salary along with the flexibility of home-based work combined with travel within the designated area.
Role Overview:
The Care Services Coordinator will oversee the management of a recovery support service designed for individuals transitioning from hospital to home.
This service operates under a model that initiates support as soon as individuals are discharged, focusing on essential aspects such as nutrition, hydration, and both cognitive and physical activities to promote health and prevent hospital readmissions.
Once individuals return home, the support continues for a period of up to six weeks, assisting them in re-engaging with their community and interests.
The primary objectives of this service include:
- Reducing recovery time post-hospitalization
- Minimizing the likelihood of readmissions
- Alleviating feelings of isolation and loneliness among older adults
The role also involves collaborating with the Service Manager in the recruitment, training, and support of volunteers, managing referrals from local partners, and ensuring compliance with service agreements and organizational values to meet performance targets.
