We are seeking a highly organized and experienced Care Coordinator to join our dynamic and friendly Care Team. As a key member of our team, you will play a vital role in supporting the delivery of outstanding home care services to our clients.

The successful candidate will be responsible for ensuring that our care services are delivered to the highest standards, while also providing exceptional support to our care professionals. If you are passionate about delivering high-quality care and have a strong commitment to customer service, we would love to hear from you.

Key Responsibilities
  • Coordinate the delivery of care services to clients, ensuring that schedules are built effectively and efficiently.
  • Support the service delivery process by responding to schedule changes and ensuring that there is sufficient cover for care assistant sickness, absenteeism, or holidays.
  • Manage client and care professional information on our care planning system, ensuring that all data is up-to-date and compliant.
  • Respond to enquiries to the service, providing warm and professional communication to clients and care professionals.
  • Collaborate with the Registered Care Manager to implement new care packages, including organizing introductions of care staff to new clients.
  • Liaise with the Care Manager and Recruitment Officer to ensure sufficient staffing levels for new and existing care packages.
  • Coordinate introductions to clients, competency checks, and support visits for the care team.
  • Support the delivery of a high-quality service through quality assurance telephone calls and visits to clients.
  • Support the recruitment and pre-employment checks for new care professionals, where appropriate.
  • Participate in the On-Call Rota and be willing to deliver care to ensure continuity of services, if required.
Requirements
  • SVQ Level 3 Qualification (or willingness to work towards it)
  • Full driving license and access to a vehicle
  • Strong IT and administrative skills
  • Team player, reliable, trustworthy, and with high integrity
  • Self-motivated, proactive, and uses own initiative
  • Impeccable attention to detail
  • A practical, methodical approach to problem-solving
  • Warm, friendly, and professional approach

What We Offer

We offer a competitive salary of £23,000 - £26,000 Pro Rota, as well as a full-time position with opportunities for growth and development. If you are passionate about delivering high-quality care and have a strong commitment to customer service, we would love to hear from you.
