We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our team as a Clinical Lead for Community Mental Health Transformation. This is an exciting opportunity to lead and develop our community mental health services, working closely with our multidisciplinary team to provide high-quality care to our patients.
Key Responsibilities
- Provide clinical leadership and management to our community mental health team, ensuring the delivery of safe, quality, and effective care.
- Develop and implement strategies to improve patient outcomes, working closely with our multidisciplinary team to achieve this goal.
- Contribute to the development of our community mental health services, identifying areas for improvement and implementing changes to enhance patient care.
- Provide education and training to our team members, ensuring they have the skills and knowledge needed to provide high-quality care.
- Lead on quality improvement initiatives, working closely with our quality team to identify areas for improvement and implement changes to enhance patient care.
Person Specification
We are looking for a highly skilled and experienced Advanced Clinical Practitioner who has:
- A Master's level award with modules in Advanced Health Assessment and Non-Medical Prescribing.
- Experience of leading and managing a multidisciplinary team.
- Experience of teaching and clinically supervising junior staff.
- Experience of assessing, formulating, and managing risk for people with severe and enduring mental health difficulties.
- Knowledge and good understanding of current trends and legislation impacting on services.
