Rheumatology and Acute Medicine Pharmacist

We are seeking a highly skilled and motivated clinical pharmacist to join our dynamic and innovative rheumatology and pharmacy teams at North Bristol NHS Trust.
Main Responsibilities- Work collaboratively with the rheumatology and acute medicine teams to further develop the service within North Bristol NHS Trust.
- Provide pharmaceutical expertise to multidisciplinary working groups considering protocols, policies, and guidelines in the specialist area.
- Manage on-going pharmaceutical care of patients being maintained on drug treatment for rheumatological and related musculoskeletal conditions.
- Provide an outpatient service to patients with autoimmune and inflammatory conditions, within own competence, referring when necessary.
- Advise on strategies to ensure cost-effective use of resources.
- Provide risk management support to the management of rheumatology patients treated at NBT.
- Produce protocols, policies, and guidelines in conjunction with the specialty teams on designated pharmaceutical practice relating to both rheumatology and acute medicine.
- Work with the specialties to comply with existing NPSA drug alerts and help with the implementation and recommendation of actions for future alerts and patient safety initiatives related to rheumatology and acute medicine.
- Provide a clinical ward pharmacy service and support to the pharmacy patient services team, which includes dispensary duties.
- Deal with pharmaceutical queries and provide advice to medical and nursing staff from both specialist areas.
- Clinically screen prescriptions to ensure the safe, efficient, evidence-based, and rational use of medicines in line with the Trust Formulary and departmental policies.
- Challenge inappropriate prescribing and escalate as appropriate.
- Support and promote antimicrobial stewardship.
- Collect clinical and cost outcome data and support both specialty teams' medicines optimisation initiatives.
- Ensure that an accurate medication history is obtained for each patient, where appropriate.
- Provide advice and information to patients/carers to improve adherence and encourage self-management regarding treatment.
- Work closely with GPs, Community Pharmacists, GP Practice Pharmacists, and other healthcare professionals across the Integrated Care System.
- Record all major interventions in line with departmental policy.
- Audit and risk manage selected areas of practice to promote continuous quality improvement.
- Develop and maintain medicines management systems in conjunction with patients/carers, ward staff, and pharmacy staff.
- Mentor junior pharmacists and contribute to the appraisal process.
- Supervise medicines management technicians and/or other dedicated pharmacy staff as required.
- Contribute to the training of pharmacy undergraduates, medical students, pre-registration pharmacists, qualified pharmacists, pharmacy technicians, and medical and nursing staff as appropriate.
- Provide patient education and clinical advice to patients in both specialties.
- Ensure that all procedures are adhered to, reviewed regularly, and that all necessary records are completed.
- Assist with establishing, implementing, and monitoring service levels of appropriate rheumatological high-cost drug therapies.
- Respond to prescription requests and queries from home delivery companies and medical teams.
- Provide intelligence and evidence of compliance requirements for PbR excluded commissioned rheumatology medication.
- Support medical teams with Individual Funding Request (IFR) submissions.
- Support medical teams with MHRA Yellow Card reporting for adverse effects related to biologics and targeted synthetic therapies.
- Provide assurance that resources are being used in line with commissioner guidance to the commissioning groups.
- Triage rheumatology non-medical prescriber (NMP) referrals from other team members.
- Undertake any other relevant work outlined by the Rheumatology Clinical Lead, Rheumatology Consultant Pharmacist, or Rheumatology Highly Specialist Pharmacist and equivalent Acute Medicine staffing.
- Deputise for the Rheumatology and Acute Medicine Highly Specialist Pharmacists if required.
- Keep up to date with developments in Pharmacy, Rheumatology, and Acute Medicine to maintain a high level of professional knowledge and competence.
- Pharmacy degree
- UK registered Pharmacist
- Previous hospital pharmacist experience
- Completed Post Graduate certificate or equivalent in Clinical Pharmacy Practice or Rheumatology
- Experience of audit, quality systems, medicines guideline or policy development
- Proactive approach to the application of specialist pharmaceutical knowledge in order to enhance patient care
- Able to manage time and prioritise effectively
- Adaptable and flexible approach to work
- Able to work under own initiative and as part of a team
- Computer literate
