Job Summary:

The ABUHB Frailty Single Point of Access (SPA) is a critical service that serves as the first point of contact for patients and medical professionals accessing community healthcare in the ABUHB area. As a Frailty Call Handler, you will play a vital role in ensuring that patients receive the best care in the most appropriate location to meet their needs.

Key Responsibilities:

  • Fielding calls from medical professionals and processing referrals for the five Gwent Community Resource Teams (CRTs) across Blaenau Gwent, Caerphilly, Monmouthshire, Newport, and Torfaen.
  • Working closely with the five CRTs and Gwent District Nursing teams to ensure that patients are referred to the most appropriate service within agreed timeframes.
  • Processing referrals from Primary Care, the Welsh Ambulance Service Trust (WAST), and other referring agencies.
  • Providing support to members of the public who require the services of a District and/or Diabetic Nurse.

Requirements:

  • Ability to work flexibly to meet service demands, including weekend working.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of community healthcare services and referral processes.
