Mental Health Assessment Specialist

Position Overview: As a key member of the Bournemouth East Collaborative Primary Care Network (PCN), you will serve as a primary contact for patients presenting with mental health concerns. Your role as a Mental Health Assessment Specialist will involve conducting initial evaluations to guide patients toward appropriate services.


Key Responsibilities:

  • Perform focused triage assessments to determine suitable next steps for patients.
  • Facilitate access to relevant mental health services, ensuring patients receive timely and appropriate care.
  • Collaborate closely with the PCN Mental Health team, including Mental Health Practitioners and lead GPs, to enhance patient care.
  • Develop strong working relationships with reception teams, GPs, and other healthcare staff within the PCN.
  • Manage administrative processes efficiently, utilizing PCN systems to support patient care.
  • Support individuals in primary care settings, working collaboratively with various agencies to address diverse mental health needs.
  • Provide mental health signposting for patients with mild to moderate mental health issues.
  • Act as the first point of contact for mental health triage, utilizing the PCN Mental Health pathway.
  • Guide patients to a variety of specialist mental health resources within the community, ensuring they receive optimal care.
  • Ensure patients are well-informed and motivated to engage with appropriate interventions and clinicians.
  • Promote social inclusion, early intervention, and recovery principles in all well-being activities.
