About the Role

This is an exciting opportunity to join Dorset Clinical Commissioning Group as a Community Mental Health Specialist. As a key member of our team, you will play a vital role in delivering high-quality mental health care services to our patients.

Key Responsibilities

  • Conduct comprehensive risk assessments and develop effective care plans to ensure the best possible outcomes for our patients.
  • Work collaboratively with multidisciplinary teams to assess, plan, implement, and evaluate programs of care.
  • Utilize excellent communication skills to engage with patients, families, and healthcare professionals, ensuring seamless care coordination.
  • Prioritize tasks, manage time effectively, and supervise, teach, and assess staff to ensure high-quality care delivery.
  • Manage complex cases with a high level of autonomy and expertise.

Requirements

  • Registered Mental Nurse with valid registration with the Nursing and Midwifery Council (NMC) or equivalent body.
  • Evidence of continuous professional development and a commitment to staying up-to-date with best practices in mental health care.
  • Proven experience in risk assessment and management, with a strong understanding of the Care Programme Approach.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Competent computer skills, with experience using electronic patient records and other relevant software.
  • Able to travel using own vehicle on Trust business, subject to the provisions of the Equality Act.
  • Demonstrable skills in written and spoken English to a standard that enables the post holder to carry out the full range of duties and responsibilities effectively.
