Community Mental Health Nurse

We are seeking a skilled and compassionate Community Mental Health Nurse to join our team in Abingdon locality. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental illnesses.
Key Responsibilities- Assess patients newly referred to the team and provide short or long-term follow-up care in accordance with their needs and goals.
- Contribute to the duty coordination rota, liaising with referrers, triaging referrals, and supporting team work and emergency assessments.
- Manage a caseload and provide evidence-based interventions to individuals with complex mental health needs.
- A supportive and collaborative team environment with opportunities for professional development and growth.
- Regular supervision and training to support your ongoing professional development.

We are looking for a candidate with:
- A Registered Professional Qualification in Mental Health Nursing or a related field.
