Senior Clinical Lead

Job Summary:
The Wirral University Teaching Hospital NHS Foundation Trust is seeking a highly skilled and experienced Senior Clinical Lead to join our Children and Young People's Continuing Care Team. As a Senior Clinical Lead, you will be responsible for providing clinical leadership and supervision to our team of Senior Clinical Support Workers and Band 5 nurse colleagues.
Key Responsibilities:
- Assessment and Planning: Identify children with complex health needs and undertake assessments under the guidance of the National Framework for Children and Young People's Continuing Care.
- Collaboration and Communication: Work in collaboration with children and young people and their families to understand their preferences in regard to care.
- Holistic Assessment: Lead on holistic assessments of the child/young person and their family.
- Lead Health Professional: Act as the Lead Health Professional to correlate reports/risk assessments from the Multi-Disciplinary Team (MDT) in respect of bespoke packages of care.
- Multi-Disciplinary Meetings: Convene and lead Multi-Disciplinary Meetings to plan care and ensure the needs of vulnerable children/families are met.
- Commissioning and Funding: Attend Children's Commissioning panel and make recommendations around funding arrangements.
- Safeguarding: Participate in safeguarding meetings in accordance with Trust and local Safeguarding Children's procedures, policies, and guidance.
- Training and Development: Be actively involved in the training process for parents, carers, and other professionals to ensure competency and evidence-based practice.
- CAF/TAC Process: Participate in the Common Assessment Framework (CAF)/Team Around the Child (TAC) process following integrated working guidelines.
