Accommodation Support Specialist (Rough Sleeping Drug and Alcohol Team)

Your Role

As a vital member of our team, you will provide essential support to clients residing in various supported accommodation facilities throughout the region.

Key Responsibilities:

  • Understand and address the unique needs of individuals at risk of homelessness.
  • Coordinate care by collaborating with housing support professionals and facilitating access to primary healthcare services.
  • Contribute to the achievement of targets set by our organization and local partners.
  • Conduct assessments, including risk evaluations and comprehensive reviews, to guide treatment pathways.
  • Engage with external agencies, including training, employment, and family support services, to ensure comprehensive client care.
  • Work alongside healthcare providers to monitor and manage client health, minimizing potential risks.
  • Encourage clients to actively participate in support services outlined in their care plans and risk assessments.
  • Assist clients with their care plans, including referrals to community support services, and provide guidance on housing, benefits, and employment opportunities.
Desirable Skills and Experience

While the following qualifications are preferred, they are not mandatory as we offer a thorough induction and continuous training to enhance our workforce:

  • Experience in substance misuse services.
  • Proficiency in conducting comprehensive assessments and developing risk management strategies.
  • Understanding of holistic care needs and community resources.
  • Commitment to promoting and supporting equality and diversity.
