The Role: Community Support Worker

We are seeking compassionate and dedicated individuals to join our Chester Outreach Support service. This vital service provides invaluable support to individuals living in the community who are facing mental health challenges.

Our team works closely with individuals, tailoring support plans to their unique needs and goals. We strive to create a supportive environment where individuals can build meaningful connections, develop essential life skills, and improve their overall well-being.

Key Responsibilities:

  • Assist individuals with daily living activities, such as managing finances, paying bills, medication management, shopping, and household tasks.
  • Facilitate social integration by encouraging participation in community activities and fostering meaningful relationships.
  • Work collaboratively with social workers and mental health professionals to ensure comprehensive care.
  • Provide emotional support and guidance to individuals navigating mental health challenges.
