About the Role

We are seeking a highly skilled and dedicated Learning Disabilities Nurse to join our team at Central London Community Healthcare NHS Trust. As a Band 6 Community Learning Disabilities Nurse, you will play a vital role in providing specialist nursing support to individuals with complex physical health needs and challenging behavior.

Main Responsibilities
  • Work as part of a multi-disciplinary team to provide high-quality care to individuals with learning disabilities, complex physical health needs, and challenging behavior.
  • Develop and implement individualized care plans to promote the health and well-being of service users.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and delivery.
  • Conduct regular reviews and assessments to identify areas for improvement and implement changes as needed.
  • Provide education and training to service users, families, and carers to promote health literacy and self-management.
Requirements
  • Qualified Learning Disability Nurse or Adult Nurse with experience working with people with learning disabilities, complex physical health needs, and challenging behavior.
  • Post-qualification experience in learning disabilities nursing in a community environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong knowledge of the Transforming Care Agenda and Learning from Mortality Reviews (LeDeR).
  • Ability to work autonomously and as part of a team, with a flexible approach to work.
