Spencer Clarke Group is seeking a skilled professional to support care leavers in their transition to independence.
This role involves managing a caseload of care leavers, developing and reviewing individual Pathway Plans, and providing advice and guidance on areas such as education, employment, housing, and health.
Key Responsibilities- Conduct Pathway Assessments in partnership with young people, social workers, carers, and other professionals.
- Assist care leavers in developing life skills for independent living, including budgeting, cooking, and managing their health.
- Ensure Pathway Plans are regularly reviewed and updated to reflect the changing needs and goals of each young person.
- Provide support in areas such as education, employment, housing, and health care for vulnerable young people.
The successful candidate will have experience working directly with children in care or young people leaving care, either as individuals or in groups.
They will also have experience in conducting Pathway Assessments and creating Pathway Plans for young people, as well as providing support in areas such as education, employment, housing, and health care for vulnerable young people.
What's on OfferSalary: £19+ph (may negotiate higher for exceptional candidates based on experience)
Contract type: 3-6 months minimum, with a high likelihood of extension
Hours: Monday to Friday, 35 hours per week
