Position Overview

We are seeking a qualified and experienced clinician for the role of Clinical Care Coordinator within our Transfer of Care Team. This is a permanent full-time position requiring flexibility to work across various days of the week during designated service hours. The team operates within a community-focused framework, necessitating travel as needed.

Key Responsibilities

  • Conduct comprehensive assessments of patient discharge needs, ensuring that care plans are developed, implemented, and evaluated based on evidence-based practices.
  • Adhere to organizational policies and procedures while executing nursing tasks, particularly in emergency situations.
  • Collaborate with a multi-disciplinary team to establish and uphold high standards of care, especially for patients with complex needs requiring coordinated interventions.
  • Maintain direct communication with patients, families, and caregivers, ensuring sensitivity to their needs for respect, dignity, and privacy.
  • Ensure meticulous documentation of discharge planning assessments, interventions, and communications.
  • Operate independently across all areas of specialty, managing personal workload to facilitate smooth and safe patient transitions.
  • Engage in the resolution and investigation of complaints related to discharge planning, taking appropriate follow-up actions.
  • Support senior nursing staff in their daily responsibilities and activities.
  • Evaluate medically stable patients for suitability for community-based care options.
  • Assist hospital wards with the completion of necessary healthcare documentation for continuing care processes.
  • Participate in daily bed management meetings as assigned.
  • Promote timely discharges from inpatient settings.

Essential Qualifications

  • NMC/HCPC registered Nurse or Allied Health Professional.
  • Clinical Leadership qualification (desirable).

  • Ability to prioritize and coordinate discharge planning effectively.
  • Competence in assessing, planning, and evaluating patient care tailored to individual needs.
  • Demonstrated enthusiasm and motivation for the clinical specialty.
  • Understanding of leadership principles and adaptability to changing circumstances.
  • Strong verbal and written communication skills.
  • Proficient in using initiative and managing stress in self and others.
  • Computer literacy (desirable).

Knowledge Requirements

  • Awareness of current nursing and allied health professional issues.
  • Understanding of evidence-based practice applications.
  • Familiarity with the Care Act 2014 and related healthcare processes.

Experience

  • Experience in various specialty areas, including acute ward settings.
  • Involvement in audit or research activities (desirable).
