Position Overview

The successful candidate will collaborate with a diverse team of healthcare professionals at St Luke's Primary Care Centre to ensure the delivery of safe, effective, and efficient medication management systems. This role is pivotal in optimizing medication use, managing repeat prescriptions, minimizing medication waste, and enhancing patient health outcomes.

As a registered healthcare professional, the Medication Management Specialist is accountable for maintaining high standards of accuracy and safety in their practice while working under the guidance of a licensed pharmacist. Continuous training and professional development will be provided to enhance knowledge in practice-based prescribing and effective medication administration, supporting the center in conducting medication reviews and achieving quality care targets.

Key Responsibilities:

- Contributing to the provision of reliable and effective pharmaceutical services.

- Conducting medication reconciliation in accordance with established protocols and standard operating procedures to enhance the efficiency and effectiveness of prescriptions.

- Addressing patient inquiries both in person and via telephone, escalating complex issues to the clinical pharmacist as necessary.

- Engaging in patient assessments and updating patient records through various methods, including clinic visits and home care.

- Verifying the accuracy of medications prescribed.

- Participating in medication audits to ensure compliance and quality.

- Offering guidance and information to physicians, staff, and patients to promote safe prescribing practices, quality, cost-effectiveness, and optimal medication use.

- Taking responsibility for maintaining a positive and organized work environment, including effective communication and data management.

- Supporting a safe working environment in line with health and safety regulations.

- Staying informed about professional responsibilities and continuing professional development (CPD) requirements.

The role involves handling sensitive patient information, which must be treated with the utmost confidentiality. All staff members are expected to respect patient privacy and adhere to policies regarding the protection of personal and sensitive data.

The Medication Management Specialist will assist in promoting health, safety, and security within the practice, including:

  • Utilizing personal security measures in accordance with practice guidelines.
  • Identifying and managing risks associated with work activities.
  • Engaging in ongoing training to enhance knowledge and skills.
  • Implementing appropriate infection control procedures and maintaining a clean work environment.
  • Reporting health and safety hazards promptly.
  • Ensuring cleanliness in work and patient areas.
  • Participating in regular infection control training.
  • Demonstrating a commitment to safeguarding and promoting the welfare of all individuals.
