The Pharmacy Technician will play a vital role in the St Luke's Primary Care Centre team, supporting the delivery of safe, effective, and efficient systems for medication optimization, repeat prescribing, reducing medication waste, and maximizing patient outcomes.
Key Responsibilities- Contribute to the provision of safe and effective pharmacy services, ensuring accurate and timely medication management.
- Undertake medicines reconciliation in line with practice protocols and standard operating procedures to improve the efficiency and effectiveness of prescriptions and repeat prescriptions.
- Respond to patients' questions and concerns, referring complex issues to the clinical pharmacist.
- Participate in patient reviews and update patient records via clinics, domiciliary visits, and in nursing and residential care homes.
- Check the accuracy of medications for prescriptions and participate in medication audits.
- Provide advice and information to GPs, staff, and patients to support the improvement of prescribing safety, quality, cost-effectiveness, and the appropriate use of medications to achieve the best outcomes for patients.
- Take responsibility for contributing to a positive and supportive working environment, including communication, data accuracy, and management.
- Support and promote a safe working environment in accordance with Health and Safety and Infection Control Policies.
- Keep up to date with professional responsibilities and CPD requirements.
