Position Overview

We are seeking a dedicated and knowledgeable Clinical Pharmacist with experience in general practice, focused on enhancing patient care through medication management.

Role Responsibilities

The Clinical Pharmacist will play a crucial role in elevating the quality and safety of healthcare services provided to patients. Key responsibilities include:

  • Conducting comprehensive medication reviews to alleviate the burden on GP prescribers.
  • Assisting in the management of patients with chronic conditions by evaluating repeat medication requests.
  • Collaborating with prescribers to ensure optimal medication prescribing, while adhering to local initiatives and maintaining financial viability.

Core Duties

The Clinical Pharmacist will be responsible for developing and managing medication optimization services within the primary care setting. The primary objectives include:

  • Reducing the medication-related workload for physicians.
  • Enhancing the safety and quality of prescribed medications.
  • Providing guidance on medication-related inquiries to the entire practice team.
  • Serving as a key contact for patients regarding medication concerns.
  • Offering senior advisory support in the daily operations of the dispensary.

Clinical Support

The Clinical Pharmacist will assist with:

  • Clinical Medication Reviews.
  • Management of chronic diseases, including hypertension, Type 2 Diabetes, Asthma, and COPD.
  • Utilizing CCG-provided clinical templates within the EMIS Web Clinical System.
  • Conducting medication reconciliation during transitions of care.

Practice Development

In addition to clinical responsibilities, the Clinical Pharmacist will focus on:

  • Improving disease registers and quality of care indicators.
  • Ensuring prescribing safety through clinical audits and support for local enhanced services.
  • Leading prescription management initiatives, ensuring proper monitoring and support for dispensary and clerical staff.

Qualifications

Essential:

  • Master's degree in Pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Membership in the Royal Pharmaceutical Society (RPS).

Desirable:

  • Postgraduate qualifications in clinical pharmacy or therapeutics.
  • Independent prescriber status.

Experience

Essential:

  • Experience in conducting medication reviews and possessing basic clinical skills.
  • A background in community pharmacy or Clinical Commissioning Groups (CCGs) with knowledge of drug budgets.
