Job Title: Clinical Pharmacist

We are seeking a highly motivated and experienced Clinical Pharmacist to join our team in General Practice. The successful candidate will have a strong background in clinical pharmacy practice, with a focus on long-term condition clinics, medication review, and medication optimisation.

Key Responsibilities:
  • Review and optimise medication regimens for patients with long-term conditions, ensuring safe and effective use of medications.
  • Conduct medication reviews with patients and provide recommendations for healthcare professionals on prescribing and monitoring.
  • Identify and mitigate risks associated with medication use, including unplanned hospital admissions and readmissions.
  • Work collaboratively with healthcare teams to ensure seamless care transitions and medication reconciliation at discharge from hospital.
  • Provide education and training to healthcare professionals on therapeutics and medication optimisation.
Requirements:
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Completed independent prescribing qualification.
  • Completed CPPE course.
  • Strong therapeutic and clinical knowledge, with a focus on long-term condition management and medication optimisation.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
