Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at The Village Practice, a dynamic and patient-centered healthcare provider in Islington London.
About the Role:
The successful candidate will work as part of a multi-disciplinary team, providing clinical leadership on medicines optimisation, quality improvement, and managing aspects of the quality and outcomes framework and enhanced services.
Key Responsibilities:
- Conduct clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, or GPs on prescribing and monitoring.
- Manage a caseload of patients with common/minor/self-limiting ailments within the scope of practice.
- Provide patient-facing clinics and a telephone helpline for patients with medicine-related queries.
- Review the use of medicines associated with unplanned hospital admissions and implement changes to reduce prescribing to high-risk patient groups.
- Ensure continuity of medicines supply to high-risk groups of patients.
- Manage the repeat prescribing reauthorisation process.
- Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
- Support public health campaigns and provide specialist knowledge on available public health programmes.
- Engage with Patient Participation Groups (PPGs) and involve them in development.
Requirements:
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Minimum of 2 years' experience as a pharmacist, demonstrated within a practice portfolio.
- Experience and awareness of common acute and long-term conditions likely to be seen in general practice.
- May hold or be working towards an independent prescribing qualification.
- Demonstrate understanding of the pharmacy role in governance and quality improvement.
- Ability to motivate self and promote diversity and equality in people management techniques.
- Willingness to travel independently between practice sites and comply with health and safety policies.
What We Offer:
We offer a competitive salary of £53,232.00-£57,360.00 per year, 37.5 hours per week, and a dynamic work environment with opportunities for professional development.
