Job Overview

We are seeking a dedicated and knowledgeable Clinical Pharmacist with experience in General Practice, focused on enhancing patient care through medication management.

The Clinical Pharmacist will play a vital role in improving the quality and safety of healthcare services provided to patients at Harford Health Centre. This position involves regular patient interactions and aims to alleviate the workload of GP prescribers by conducting thorough medication reviews. Additionally, the pharmacist will assist in managing patients with chronic conditions by evaluating repeat medication requests.

Main Responsibilities

The Clinical Pharmacist will:

  • Develop and oversee medication optimization services within the primary care setting.
  • Reduce the medication-related workload for physicians.
  • Enhance the safety and quality of prescribed medications.
  • Provide guidance on medication-related matters to the entire practice team.
  • Act as a primary contact for patients regarding medication inquiries.
  • Serve in a senior advisory capacity for the daily operations of the dispensary.

Key clinical responsibilities include:

  • Conducting Clinical Medication Reviews.
  • Managing Chronic Diseases (e.g., hypertension, Type 2 Diabetes, Asthma, COPD, polypharmacy reviews).
  • Utilizing appropriate clinical templates within the EMIS Web Clinical System.
  • Performing medication reconciliation during transitions of care.

Additionally, the pharmacist will support practice development initiatives, focusing on:

  • Quality and Outcomes Framework (QoF) Domains and enhancing disease registers.
  • Prescribing safety, clinical audits, and local enhanced services.
  • Leading prescription management processes and ensuring effective monitoring.

Harford Health Centre operates from a modern, purpose-built facility. This is an excellent opportunity to be part of a progressive and busy General Practice, with a dispensing volume of approximately 11,000 items monthly and a growing patient list.

Qualifications

Essential:

  • Master's degree in Pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Membership in the Royal Pharmaceutical Society (RPS).

Desirable:

  • Postgraduate qualifications in clinical pharmacy, community pharmacy, or therapeutics.
  • Independent prescriber status.

Experience

Essential:

  • Additional qualifications in chronic disease management or minor ailments.
  • Experience in conducting medication reviews and possessing fundamental clinical skills.
  • A background in community pharmacy or Clinical Commissioning Groups (CCGs) with experience in drug budgets and switches.
