Job Overview

We are seeking a dedicated and knowledgeable General Practice Pharmacist with substantial experience, focused on enhancing their expertise to elevate patient care standards.

The Clinical Pharmacist will play a crucial role in improving the quality and safety of healthcare services provided to patients at Harford Health Centre. This position involves regular interactions with patients and aims to alleviate the workload of GP prescribers by conducting thorough medication reviews. Additionally, the pharmacist will assist in managing patients with chronic conditions through the evaluation of repeat medication requests.

Key Responsibilities

The Clinical Pharmacist will be responsible for developing and overseeing medication optimization services within primary healthcare settings.

The primary objectives for this role include:

  • Reducing the medication-related workload on physicians
  • Enhancing the safety and quality of prescribed medications
  • Providing comprehensive support on medication-related matters to the entire practice team
  • Serving as a resource for patients regarding medication inquiries
  • Assuming a senior advisory role in the daily operations of the dispensary

The Clinical Pharmacist will support various clinical activities, including:

  • Conducting Clinical Medication Reviews
  • Managing Chronic Diseases (e.g., hypertension, Type 2 Diabetes, Asthma, COPD, polypharmacy assessments)
  • Utilizing CCG-provided clinical templates within the EMIS Web Clinical System
  • Performing medication reconciliation during transitions between care providers

Additionally, the pharmacist will contribute to practice development initiatives, focusing on:

  • Quality and Outcomes Framework (QoF) Domains and enhancing disease registers
  • Prescribing Safety, Clinical Audits, and support for Local Enhanced Services
  • Leading prescription management, ensuring proper monitoring, and supporting dispensary and clerical staff

About Harford Health Centre

Harford Health Centre operates from a purpose-built facility, representing an exciting opportunity to join a progressive and modern General Practice. The practice is busy, dispensing approximately 11,000 items monthly, with a patient list of 10,000 that is expected to grow as new residential developments occur in the area.

Qualifications

Essential:

  • Master's degree in Pharmacy (MPharm) or equivalent
  • Registered with the General Pharmaceutical Council (GPhC)
  • Membership in the Royal Pharmaceutical Society (RPS)

Desirable:

  • Postgraduate qualifications in clinical, community, or therapeutics
  • Independent prescriber status

Experience

Essential:

  • Additional qualifications in chronic disease management or minor ailments are advantageous
  • Experience in conducting medication reviews and possessing basic clinical skills
  • A background in community pharmacy or Clinical Commissioning Groups (CCGs) with experience in drug budgets and switches
