About the Service - Housing & Homelessness Services

A safe home is the foundation upon which individuals can build their lives. We provide support to people experiencing homelessness or at risk of becoming homeless, as well as specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women, and young people.

Our approach to accommodation goes beyond providing a roof over one's head and shelter from the elements. We strive to create a welcoming and flourishing environment that offers opportunities for our clients to feel comfortable and develop. You can make a meaningful difference in the lives of our clients by supporting them on their journey towards a brighter future.

About the Job

We are seeking a highly motivated and passionate individual to join our team at The Fells. As a Hospital Discharge Support Coordinator, you will work collaboratively with the hospital discharge team and serve as a link between receiving referrals from the team into our service.

You will have supervisory responsibilities, including line managing a move-on worker based in The Fells and part of our mental health hub. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
