About the Service - Housing & Homelessness Services

About the Job

We are seeking a highly motivated and passionate individual to join our team at The Fells. As a Hospital Discharge Support Coordinator, you will work collaboratively with the hospital discharge team and serve as a link between referrals from the team into our service.

You will have supervisory responsibilities, including line managing a move-on worker based in The Fells and part of our mental health hub. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
