About the Service - Housing & Homelessness Services
A safe home is the foundation upon which individuals can build a brighter future. We provide specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women, and young people. Our goal is to create a welcoming and flourishing environment that offers opportunities for growth and development.
About the Role
We are seeking a highly motivated and compassionate individual to join our team as a Hospital Discharge Support Coordinator. As part of our vibrant team, you will work collaboratively with the hospital discharge team and be the link between receiving referrals from the team into our service. You will also have supervisory responsibilities, line managing a move-on worker based in The Fells and part of the mental health hub.
