About the Role

We are seeking a dedicated and passionate individual to join our team as a Support Coordinator for Hospital Discharges. In this role, you will work closely with the hospital discharge team to provide support and assistance to individuals in need.

As a Support Coordinator, you will be responsible for supervising a move-on worker and collaborating with the hospital discharge team to ensure seamless referrals into our service.

  • Supervise a move-on worker to ensure successful placement and support of individuals in our service.
  • Work collaboratively with the hospital discharge team to receive referrals and ensure a smooth transition into our service.
