About the Service - Housing & Homelessness ServicesWe provide specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women, and young people. Our goal is to provide a welcoming and flourishing environment that offers opportunities for all those we support to feel comfortable and develop.
We offer a home rather than just a shelter, providing a safe and supportive space for individuals to rebuild their lives.
About the JobWe are seeking a highly motivated and passionate individual to join our team at the Fells. As a Hospital Discharge Support Coordinator, you will work collaboratively with the hospital discharge team and be the link between receiving referrals from the team into the Fells service.
You will work as part of an enthusiastic support team and work alongside a mental health hub, which consists of Occupational Therapists and Mental Health Engagement Workers. As part of the role, there will be supervisory responsibilities of line managing a move-on worker based in The Fells and part of the mental health hub.
