We are seeking a highly motivated and compassionate individual to join our team as a Stroke Support Coordinator. This is an exciting opportunity to work with stroke survivors and their families, providing personalized support and guidance to help them navigate their recovery journey.

Key Responsibilities
  • Support new stroke survivors and their carers from hospital discharge into the community
  • Provide tailored information, advice, and support to help clients make informed lifestyle changes
  • Collaborate with other organizations to achieve joint ambitions and improve stroke care
Requirements
  • Experience in a caring profession, ideally with a background in supporting people with disabilities
  • Ability to work collaboratively with others and demonstrate excellent communication and organizational skills
  • Basic Microsoft system skills and ability to use technology to support client engagement
