About the Service - Housing & Homelessness Services

About the Job

We are seeking a highly motivated and passionate individual to join our team at The Fells. As a Hospital Discharge Support Coordinator, you will work collaboratively with the hospital discharge team and be the link between receiving referrals from the team into our service.

You will have supervisory responsibilities, including line managing a move-on worker based in The Fells and part of our mental health hub. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
