Job Overview

We are seeking a highly skilled and compassionate Stroke Support Coordinator to join our team in the Durham area. As a key member of our Stroke Recovery Service, you will play a vital role in supporting stroke survivors and their families to rebuild their lives.

The successful candidate will have a strong background in a caring profession, with experience of supporting individuals with disabilities. You will be able to demonstrate your ability to work collaboratively with other organisations to achieve joint ambitions and possess excellent communication and organisational skills.

Key Responsibilities
  • Support new stroke survivors and their carers from hospital discharge into the community, providing personalised information, advice, and support.
  • Help clients make informed lifestyle changes to prevent further strokes, working collaboratively with healthcare professionals and other stakeholders.
  • Conduct home visits and community outreach to provide support and guidance to stroke survivors and their families.
About You

We are looking for a motivated and empathetic individual who is passionate about supporting stroke survivors and their families. You will have a strong understanding of the challenges faced by stroke survivors and their carers, and be able to provide tailored support to meet their individual needs.

You will be able to demonstrate your ability to work independently and as part of a team, with excellent communication and interpersonal skills. A strong background in a caring profession, with experience of supporting individuals with disabilities, is essential for this role.
