Support Coordinator

We are seeking a highly motivated and organized individual to join our team as a Support Coordinator. This is an exciting opportunity to work with stroke survivors and their families to provide support and guidance following a stroke.

About the Role
As a Support Coordinator, you will be responsible for providing emotional and practical support to stroke survivors and their families. You will work closely with our team to develop and implement personalized support plans, ensuring that each individual receives the care and support they need to achieve their goals.

Key Responsibilities
  • Provide emotional and practical support to stroke survivors and their families
  • Develop and implement personalized support plans
  • Work closely with our team to ensure seamless delivery of support services
  • Communicate effectively with stroke survivors, families, and healthcare professionals
  • Maintain accurate records and reports

Requirements
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team
  • Empathy and understanding of the needs of stroke survivors and their families
  • Ability to maintain confidentiality and handle sensitive information
  • Basic IT skills and ability to learn new systems
