We are looking for enthusiastic and committed professionals to fill both permanent and temporary positions within our District Nursing Service. Successful candidates will find themselves in a nurturing environment that fosters the development of advanced skills in patient assessment and care, while also contributing to the leadership dynamics of the team.

Candidates must be prepared to work flexible hours, including rotational shifts across day and night, as required by service demands. This role supports a 7-day service, inclusive of weekends and public holidays.

Key Responsibilities

In this role, you will assist the Clinical Team Leader in coordinating and delivering care in alignment with both national and local District Nursing strategies.

  • Conduct assessments for new and complex patients
  • Deliver comprehensive Senior Nurse Reviews in various community settings
  • Collaborate as part of a multidisciplinary team
  • Mentor students and new staff in their professional development
  • Foster team growth through effective leadership and role modeling
  • Manage staffing schedules and allocate clinical responsibilities
  • Maintain clear communication with clinical staff, patients, and their families
  • Provide weekend, evening, and holiday cover as part of the District Nursing Hub (on a rotational basis)
  • Adapt to working contracted hours across day and night shifts as needed

Problem-solving skills and adaptability to challenging situations are essential, along with efficient resource management to ensure the delivery of safe, personalized, and effective patient care.   • Community Specialist Practitioner Qualification or willingness to pursue this qualification
