Job Summary

The North Staffordshire Combined Healthcare Trust is seeking a highly skilled Community Mental Health Nurse to join our team as a Care Coordinator. This is an exciting opportunity to work with a dedicated team of professionals who are passionate about delivering high-quality care to our patients.

Main Responsibilities
  • Act as a named mental health Nurse/care co-ordinator with responsibility for the assessment, planning, and delivery of care to the identified patient group.
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
  • Conduct thorough assessments and develop personalized care plans that meet the unique needs of each patient.
  • Collaborate with the multidisciplinary team to ensure that patients receive comprehensive and coordinated care.
  • Participate in the development and implementation of care pathways and protocols to improve patient outcomes.

Person Specification
  • Registered Mental Health Nurse with a valid registration with the NMC.
  • Experience in Adult and/or Older Adult Mental Health.
  • Experience of working therapeutically in community settings.
  • Experience of working with a range of statutory, non-statutory agencies.
  • Knowledge/awareness of relevant legislation and guidance including common law issues, the Mental Health Act (1983), Community Care Act (1990), Human Rights Act (1998), National Service Framework, and Clinical Governance.
  • Understanding of psychosocial interventions.
