About the Role

We are seeking a highly motivated and experienced Community Health Nurse to join our Crisis Team in Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality care and treatment to patients in their own homes.

Key Responsibilities
  • Deliver care and treatment to patients with mental health crises in their own homes.
  • Conduct risk assessments and develop safety plans to ensure patient safety.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care.
  • Engage with patients, carers, and other professionals to provide support and guidance.
  • Participate in daily team reviews and contribute to the development of care plans.
Requirements
  • First-level registered nurse qualification (RNMH, RMN, RNLD, or equivalent).
  • 12 months post-registration experience and completed preceptorship.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
