Job Summary

The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly motivated and experienced nurse to join our Universal Crisis Team. As a Community Nurse, you will play a vital role in providing high-quality care and treatment to patients in their homes, working closely with a multidisciplinary team to deliver a 7-day-a-week service.

Main Responsibilities
  • Deliver care and treatment to patients with mental health crises in their homes
  • Offer various therapeutic interventions and ongoing risk management
  • Participate in daily multidisciplinary team reviews and liaise with other professionals

Person Specification
  • Essential:
    • Risk Assessment and Risk Management/Safety Planning skills
    • Well-developed inter-personal skills with ability to consult, liaise and negotiate with service users, carers, other professionals and wider community
    • First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification Social Worker, OT, other
    • 12 months post registration experience, and completed preceptorship
  • Desirable:
    • Experience of working within a community team
    • Care planning and risk management planning
    • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post
