Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking an experienced and skilled nurse to join our Community Nursing team as a District Community Nursing Sister. Main Responsibilities
  • Provide leadership and management to a team of community nurses, ensuring the delivery of high-quality patient care and services.
  • Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals to achieve optimal outcomes.
  • Manage a complex and diverse caseload, prioritizing tasks and resources to ensure efficient and effective service delivery.
  • Collaborate with other teams and services to ensure seamless care transitions and coordination.
  • Contribute to the development and implementation of policies, procedures, and guidelines to improve patient care and service delivery.
Requirements
  • Registered Nurse - Adult qualification and current NMC registration.
  • Experience in community nursing, with a strong understanding of the role and responsibilities of a District Community Nursing Sister.
  • Leadership and management experience, with the ability to motivate and inspire a team to deliver high-quality patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care and services.
