Key Responsibilities

  • Serve as a Named Worker for a designated caseload, ensuring service users have a consistent point of contact and fostering their independence.
  • Collaborate with the Multidisciplinary Team (MDT) to support the care and treatment plans of individuals.
  • Engage with service users to provide care and treatment, adjusting plans as necessary in response to changes in their mental or physical health.
  • Contribute to the planning and review of Recovery Goals with service users and team members.
  • Maintain a thorough understanding of care and support planning.
  • Assist in the completion of Patient Reported Outcome Measures (PROMs) to support care planning and reviews.
  • Adhere to service policies regarding confidentiality and information sharing.

Additional Duties

  • Participate in Multi-Disciplinary Meetings and daily briefings.
  • Provide unplanned activity support through same-day appointment slots.
  • Engage in peer supervision and receive regular planned supervision.
  • Support the development of students and contribute to their learning experiences.
  • Commit to ongoing professional development and ensure compliance with mandatory training.
  • Maintain accurate records of all interactions with service users.
  • Perform any other duties as assigned by the Team Manager within the scope of the role.

  • Familiarity with current issues affecting individuals with serious mental health conditions.
  • Experience working within Community Mental Health Teams.
