Position Overview

The Family Support Coordinator (FSC) plays a vital role in fostering connections with families and enhancing collaborations within the community. This position is tasked with maintaining accurate records in the program's data management system to reflect family involvement. The Family Support Coordinator will serve as a representative of the organization in designated areas to achieve impactful outcomes.

Key Responsibilities:

  • Facilitate opportunities for families to engage in the Family Partnership Agreement goal-setting process.
  • Develop and uphold a tracking system for Family Partnership Agreements to ensure all families have the chance to set objectives.
  • Collaborate with parents to complete the Family Outcome Framework.
  • Organize and lead School Readiness Transition Meetings for parents.
  • Monitor classroom attendance on a weekly basis.
  • Follow up with families regarding children with frequent absences.
  • Provide a monthly report on attendance for children below 85% participation.
  • Engage with healthcare providers and community partners during on-site screenings and assessments for enrolled children.
  • Assist families in navigating the application process for medical insurance.
  • Participate in the execution of the organization's Parent Orientation, either remotely or in person.
  • Attend Policy Council Meetings at least once during the academic year, either remotely or in person.
  • Collaborate with families within 30 days of a child's enrollment to verify ongoing access to healthcare and document findings.
  • Complete mandated health screenings within 45 days of a child's enrollment.
  • Accurately document health screening results in the data management system.
  • Work with families within the first 90 days of enrollment to confirm if the child is up-to-date on preventive medical and dental care.
  • Record all interactions with families regarding the acquisition of necessary health documentation.
  • Distribute, review, and monitor In-Kind contributions from families.
  • Validate and accurately enter In-Kind contributions in the data management system.
  • Ensure all required Program Information Report (PIR) documentation is entered correctly.
  • Recruit volunteers to assist the organization in meeting In-Kind match requirements.
  • Identify and recruit eligible children and families for the program.
  • Document recruitment efforts thoroughly.
  • Accurately complete and enter intake applications in the data management system.
  • Communicate with Child Services Administration regarding parent representation in Policy Council meetings.
  • Participate in the Health Services Advisory Committee as needed.
  • Assist in recruiting potential substitutes to maintain appropriate staff-child ratios in classrooms.
  • Support classroom ratios as needed upon request from the Supervisor.
  • Engage in and/or coordinate community outreach initiatives.
  • Empower families to advocate for their child's services.
  • Perform additional tasks as assigned by the Center Director to ensure the needs of children and families are met.

  • Undertake other duties as necessary to fulfill the responsibilities of the Family Support Coordinator position.

Work Relationships and Scope:

This position reports directly to the Center Director and involves regular interactions with families, children, staff, and local community resource providers.

Performance Metrics:

  • Establish and maintain relationships with enrolled families (Ongoing).
  • Ensure 100% of Family Partnership goals established with families are SMART (Ongoing).
  • Document 90% of follow-ups for children with attendance below 85% (Weekly).
  • Develop a plan with 85% of families whose children have missed 10% of program days (Ongoing).
  • Achieve 95%-100% accuracy in data management system documentation (Daily).
  • Submit weekly performance reports to the supervisor (100% compliance).
  • Complete mandated health screenings within guidelines (100% compliance).
  • Ensure a Policy Council Parent Representative is present (100% compliance).
  • Confirm participation of the parent Policy Council Representative in scheduled meetings (100% compliance).
  • Maintain active Parent Center Committees with accessible documentation (100% compliance).

Required Knowledge, Skills, and Abilities:

A Bachelor's Degree in Social Work or a related field is required, along with the ability to obtain a Family Development Credential within 18 months of hire. The candidate should possess problem-solving skills and the ability to work collaboratively in a team environment. Cultural humility and sensitivity are essential, along with excellent communication skills. Proficiency in technology is necessary, as is the ability to implement relevant performance standards and licensing requirements. The candidate must be capable of working independently to achieve measurable family outcomes.

Working Conditions:

The role is primarily office-based with minimal safety concerns. Due to health considerations, remote work may be requested and requires supervisor approval. Frequent travel to designated centers and participation in internal and external meetings, training sessions, and community activities are expected. The position involves sitting, standing, bending, and minimal lifting. A valid driver's license and a safe driving record are required, along with the ability to travel by air. Successful completion of background checks is mandatory.
