Children's Outreach Nurse

We are seeking a highly skilled and experienced Acute Children's Outreach Nurse to join our team at University Hospitals Sussex NHS Foundation Trust. As a key member of our Acute Children's Outreach Nursing Service (ACORNS), you will play a vital role in providing high-quality, patient-centered care to infants, children, and young people in their own homes.
Main Responsibilities:- Conduct comprehensive, evidence-based clinical assessments of children in their own homes, using robust assessment tools and digital programmes.
- Develop and implement individualized care plans, working in partnership with children, young people, and their families to empower them to manage their acute care needs.
- Provide expert nursing care, including physical assessments, emergency interventions, and monitoring of acute exacerbations of illness.
- Collaborate with acute hospital colleagues to ensure timely and safe discharge from hospital and effective follow-up in the community setting.
- Participate in health promotion and education, contributing to the well-being of patients and their families.
- Registered Nurse - Child Branch or Adults nurses with Conversion certifications.
- Experience working with children in an Acute inpatient or acute community setting.
- PILS.
- Substantial Experience working within Children's Emergency nursing or a Hospital at home service.
- Children's ED course, Physical assessment course or Equivalent.
- Teaching and assessment in clinical practice or equivalent.
- APLS/EPLS provider or Equivalent.
