We are seeking a highly skilled and experienced Paediatric Nurse to join our Acute Children's Outreach Nursing Service (ACORNS) team. As an Acute Children's Outreach Nurse, you will play a vital role in providing high-quality, patient-centered care to children and families in their own homes.
Key Responsibilities- Conduct structured clinical telephone and video assessments to identify the needs of children on the ACORNS caseload.
- Provide holistic, autonomous, and clinical nursing services using evidence-based practice, including initial assessments, home visits, and planning, implementing, and evaluating nursing care.
- Work in partnership with children and families to assess their needs and plan care, providing advice and contributing to the well-being of patients.
- Recognize and respond to deteriorating children, initiating nursing and emergency interventions as needed.
- Monitor acute exacerbations of illness and initiate appropriate nursing interventions, ensuring clinical practice is based on current local and national guidelines.
- Initiate referrals and work autonomously with children and families in the community setting.
- Provide structured clinical assessments of individual needs, empowering children, young people, and parents to support their acute care needs.
- Liaise with acute hospital colleagues to enable timely and safe discharge from hospital and appropriate follow-up in the community setting.
- Registered Nurse - Child Branch or Adults nurses with Conversion certifications.
- Experience working with children in an Acute inpatient or acute community setting.
- PILS.
- Acute nursing skills.
- Substantial experience working within Children's Emergency nursing or a Hospital at home service.
- Children's ED course, Physical assessment course, or Equivalent.
- Previous Autonomous Practice experience.
- Teaching and assessment in clinical practice or equivalent.
- APLS/EPLS provider or Equivalent.
- Evidence of having championed diversity in previous roles.
- Evidence of having undertaken own development to improve understanding of equalities issues.
