Job Summary

The Acute Children's Outreach Nursing Service (ACORNS) is a pioneering initiative at University Hospitals Sussex NHS Foundation Trust, formed of highly experienced Paediatric nurses who provide expert care to infants, children, and young people during acute episodes of illness. Our service aims to support patients and families by preventing unnecessary hospital admissions, facilitating early discharge, reducing re-attendance to hospital, and providing an outstanding patient experience.

We work closely with the Children's Emergency Department (CED) and other clinical teams to ensure seamless care coordination and effective communication. Our team uses robust assessment tools and digital programmes to deliver high-quality care, and we provide advice, support, education, and health promotion to families with every interaction.

Main Duties

As an Acute Children's Outreach Nurse, you will provide evidence-based and structured clinical telephone and video assessments to children on our caseload. You will use your highly competent communication skills to assess, monitor, and plan care, providing consistency, continuity, and advice to families at home.

You will work autonomously with the child and family in the community setting, initiating appropriate work, accepting referrals, and ensuring all your clinical practice is based on current local and national guidelines for the assessment and recognition of the sick child.

Person Specification

We are looking for a Registered Nurse with experience working with children in an acute inpatient or community setting. You will have PILS and acute nursing skills, and a strong understanding of equality, diversity, and inclusion. Substantial experience working within Children's Emergency nursing or a Hospital at Home service is desirable, as is a Children's ED course or equivalent.
