About the Role

We are seeking an experienced and skilled Acute Children's Outreach Nurse to join our team at University Hospitals Sussex NHS Foundation Trust (279). As a key member of our community care team, you will be responsible for providing high-quality, patient-centered care to children and families in their own homes.

Key Responsibilities
  • Conduct comprehensive, evidence-based assessments of children's needs and develop personalized care plans to support their acute care requirements.
  • Provide expert clinical guidance and support to families, empowering them to manage their child's condition effectively.
  • Work collaboratively with multidisciplinary teams, including acute hospital colleagues, to ensure seamless transitions of care and timely discharges.
  • Develop and maintain strong relationships with families, building trust and confidence in our community care services.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in paediatric care, applying this knowledge to inform your practice and improve patient outcomes.
Requirements
  • Registered Nurse - Child Branch or Adults nurses with Conversion certifications.
  • Experience working with children in an Acute inpatient or acute community setting.
  • PILS.
  • Acute nursing skills.
Desirable Qualifications
  • Substantial Experience working within Children's Emergency nursing or a Hospital at home service.
  • Children's ED course, Physical assessment course or Equivalent.
  • Previous Autonomous Practice experience.
  • Teaching and assessment in clinical practice or equivalent.
  • APLS/EPLS provider or Equivalent.
