The Acute Children's Outreach Nursing Service (ACORNS) is a dynamic team of experienced Paediatric nurses who provide comprehensive care to infants, children, and young people in their own homes. Our service aims to prevent unnecessary hospital admissions, facilitate early discharge, and reduce re-attendance to hospital. We work closely with families, healthcare professionals, and other services to deliver high-quality care and support.
Main Responsibilities- Provide evidence-based, structured clinical telephone and video assessments to children on the ACORNS caseload.
- Carry out initial assessments, home visits, and plan, implement, and evaluate nursing care for patients with continuing responsibility.
- Act in partnership with the child and family to assess needs and plan care, providing advice contributing to the well-being of the patient and participating in health promotion.
- Monitor acute exacerbations of illness and initiate appropriate nursing interventions, ensuring all clinical practice is based on current local and national guidelines.
- Initiate appropriate work, accepting referrals, and working autonomously with the child and family in the community setting.

Person Specification- Registered Nurse - Child Branch or Adults nurses with Conversion certifications.
- Experience working with children in an Acute inpatient or acute community setting.
